We are pleased to announce, as part of the Affordable Care Act (sometimes called Obamacare), you may be eligible for AHCCCS benefits (Arizona Health Care Cost Containment System, Arizona’s Medicaid Plan) or another insurance effective January 1, 2014.
CODAC will be able to assist you in applying for AHCCCS or in a referral to apply for other plans beginning October 22, 2013. It is very important that you come in as soon as possible to apply for these new benefits.
To apply for AHCCCS or another insurance plan:
Come to CODAC at Alvernon, located at 630 N. Alvernon (Northeast corner of Alvernon & Fifth Street) and tell the Front Office staff you are here to apply for benefits. Staff will be available to assist you during the following days and hours.
October 23 through December 18
Wednesdays from 8:00 a.m. – 6:00 p.m. (You must arrive by 6:00 p.m.)
October 26, November 16, and December 7
Saturdays from 10:00 – 1:00 p.m.
Please note, CODAC will be closed for the following Holidays/Events:
- Monday, November 11 for Veterans Day
- Thursday, November 28 and Friday, November 29 for Thanksgiving
- Thursday, December 12, 2:30-5:00 p.m. for All Staff Event
- Wednesday, December 25 for Christmas
- Wednesday, January 1 for New Year’s Day
