Welcome to CODAC Health, Recovery & Wellness. We are here to help you meet your healthcare needs. As a new client – we refer to our clients as “members” – here is some information that might help you get acclimated to CODAC.
What to Expect at Your Treatment Center
The types and amount of care you receive depends on your individual needs and your insurance coverage or ability to pay. Your first appointment will usually be an assessment by one of our trained staff.
During your initial assessment (at your first appointment), you will talk with staff and professionals about your main concerns, treatment goals and how CODAC can help you. You will be asked detailed questions about your background, treatment history and current symptoms. This appointment can take anywhere from 60 to 90 minutes. You may also meet other staff who you will be on your treatment team.
Planning Your Care
During your assessment, you will work with our staff to develop a treatment plan, called an Individual Service Plan (ISP). Your ISP will consist of a list of your goals for treatment, your strengths, your identified needs, and what covered behavioral health services and informal supports will be used to help you meet your goals.
Services outlined in your Individual Service Plan may include:
- Psychiatric assessment
- Psychiatric care and medication management
- Group, family and individual therapy
- Referral and case management
- Interaction with a Peer Support Specialist
- Wellness goals
- Physical health goals & connection with a primary care provider
We offer a variety of therapy groups across all CODAC sites. CODAC members are welcome to participate in groups at any outpatient treatment center, as long as it is authorized by your insurance and treatment team. Individual, couples and family therapy is also available.
If you are in need of medication for your behavioral health issues, you will see a Behavioral Health Medical Professional (BHMP) located at your service site. The Behavioral Health Medical Professional is the Psychiatrist, Doctor of Nursing Practice or Nurse Practitioner who oversees the prescribing of medications and makes sure the medications are working well for you and your recovery. They also check in with your overall health and wellness and review other related concerns you may be experiencing.
At CODAC, our new integrated health care approach combines primary health care with the mental health, addiction and trauma services we’re known for.
Our experts - including primary care doctors, psychiatrists, nurses, medical assistants, therapists, case managers and peer support specialists - work together as one team in one location. The result: highly personalized care for the whole you.
Alongside out treatment for substance abuse, anxiety, depression and other mental illnesses, we can now help you with common illnesses and injuries, physicals and screenings, and chronic conditions like diabetes and high blood pressure.
Wellness support specialists are also available to lead workshops and classes on a variety of wellness topics and to help you make a personalized wellness plan.

* We will do our best to accommodate your intake appointment on the same day. However, situations may arise in which all intake staff are occupied or there may not be enough time to complete an intake appointment if you arrive later in the day. In these situations, you may be asked to return for your appointment on the next business day.
