Billing and Insurance

Fees and financial obligations for services received at CODAC vary greatly depending on:

  • The type(s) of service you receive
  • The types of treatment and programs you participate in
  • Your AHCCCS-eligibility (Medicaid)
  • Your behavioral health diagnosis
  • Your income level

During your first appointment - and as needed - you will meet with a Membership Specialist to review your behavioral health care benefits and any fees or financial obligations you may have at CODAC.

Paying Out-of-Pocket

If you don't qualify for AHCCCS and either don't have insurance or your insurance doesn't cover services at CODAC, you can take advantage of CODAC's sliding scale fee-for-services. Our rates are reasonable and enable you to choose from an "a-la-carte" style menu of services based on your needs and your budget.

CODAC does not deny services based on a person's race, color, sex, gender, age, national origin, disability, religion, sexual orientation or inability to pay.
