Billing and Insurance

Fees and financial obligations for services received at CODAC vary greatly depending on:

  • The type(s) of service you receive
  • The types of treatment and programs you participate in
  • Your AHCCCS-eligibility (Medicaid)
  • Your behavioral health diagnosis
  • Your income level

During your first appointment - and as needed - you will meet with a Membership Specialist to review your behavioral health care benefits and any fees or financial obligations you may have at CODAC.

Insurance Coverage

CODAC participates with the following insurance insurance carriers for many products and services. However, not all doctors, nurse practitioners and therapists are contracted with each plan. Please ask to speak with one of our Membership Specialists who can see if the provider you want to work with is contracted with your insurance.
