Dr. Sabreen Boone is a family medicine provider with over 11 years of experience practicing medicine. She has also held progressively responsible leadership roles during her career, including owning and operating a concierge medical practice in Tucson for the past five years.
“I am proud to be joining CODAC’s integrated care treatment model. As a primary care provider, over 70% of visits are mental health related,” Dr. Boone says. “It’s such a benefit to the patient and the providers to have an integrated practice like we do at CODAC. Physical care, mental health and wellness services in one location improve health outcomes for our community’s most vulnerable.”
Dr. Boone received her Doctor of Osteopathy (DO) degree from the University of Medicine and Dentistry of New Jersey School of Osteopathic Medicine. She also completed a Family Medicine Residency and Integrative Medicine Fellowship at the University of Arizona Department of Family and Community Medicine. She is currently pursuing her Masters of Medical Management from the University of Arizona Eller School of Business. She is a member of the American Association of Family Physicians and the American Osteopathic Association.
As Associate Medical Director at CODAC, Dr. Boone is responsible for providing high quality care; establishing policies and procedures and recommending changes to improve patient experience and outcomes; supervising and evaluating the work of medical staff; and working collaboratively to further develop CODAC’s integrated care model and services, among other critical functions.
