Coverage for Services

Fees and financial obligations for services received at CODAC vary greatly depending on:

  • The type(s) of service(s) you receive
  • The types of treatment and programs you participate in
  • Your AHCCCS eligibility (Medicaid)
  • Your behavioral health diagnosis
  • Your income level

A membership specialist will contact you to review your healthcare benefits and any fees or financial obligations you may have. They can also help you apply for AHCCCS (Arizona’s Medicaid) and determine if you qualify for any grant funding.  

Participating Insurance

CODAC participates with the following insurance carriers for many products and services. However, not all doctors, nurse practitioners and therapists are contracted with each plan. Please ask to speak with one of our membership specialists who can see if the provider you want to work with is contracted with your insurance.

Paying Out-of-Pocket

CODAC has a discounted/sliding fee schedule based on family size and income. If you don’t qualify for AHCCCS and either don’t have insurance or your insurance doesn’t cover services at CODAC, you can take advantage of CODAC’s discounted/sliding fee schedule. Our rates are reasonable and enable you to choose from an “a-la-carte” style menu of services based on your needs and your budget. No one will be denied access to services due to inability to pay.

CODAC does not deny services based on a person’s race, color, sex, gender, age, national origin, disability, religion, sexual orientation, gender identity, Medicare, Medicaid, or Children’s Health Insurance Program (CHIP) coverage or inability to pay.
