Our Holistic Approach to Integrated Care

CODAC’s integrated health model is a comprehensive, holistic approach to care and wellbeing that focuses on the whole person and your family/family of choice.

A diverse mix of health and wellness professionals work together to provide unified care to individuals, couples and families, using evidence-based and best practice treatment methods.

Because wellness is achieved over time, CODAC works with members beyond their short-term needs to help them meet their ongoing health and wellness goals.

CODAC’s integrated care model improves health outcomes and achieves optimal wellness by providing exceptional integrated care, providing health education, promoting disease prevention and management, and improving overall health and wellness.

We believe it is important to address the full spectrum of physical, emotional, behavioral, social, spiritual, relational and environmental factors that impact wellness.
